Community Health Plan of Washington Individual & Family Plans is a health insurance plan for individuals and families. To enroll in a Community Health Plan of Washington Individual and Family plans, you must be eligible and live in our service area (Adams, Asotin, Benton, Chelan, Clallam, Columbia, Douglas, Ferry, Franklin, Grant, Jefferson, King, Kitsap, Kittitas, Lewis, Lincoln, Mason, Okanogan, Pierce, Snohomish, Spokane, Stevens, Thurston, Walla Walla, Whitman, and Yakima counties in Washington state).
