Your Member ID card is your health care connection
You need to show your Community Health Plan of Washington (CHPW) Member ID card when you get health care services or fill prescriptions. If you don’t have your card or have lost it, you can log into the CHPW Member Center through your phone or computer to download and/or print a copy.
Note: New members can create a CHPW Member Center account after their effective date of coverage (the date that coverage is officially active).
If you do not have your ID card, our in-network providers, hospitals, and pharmacies can confirm your coverage directly with us.

I just enrolled. When will I get my member ID card?
You will receive your ID card within 30 business days of enrollment. You can also print a copy from the CHPW Member Center.
Is there a separate ID card for prescription drugs?
Your CHPW ID card is the only card you need. It can be used for medical and prescription drug coverage.
My information is incorrect. How do I fix it?
If the information is incorrect on your current ID card, please call Customer Service at 1-866-907-1906 (TTY Relay: Dial 711) from 8:00 a.m. to 5:00 p.m., Monday to Friday. Once your information has been updated, you can ask Customer Service to send you a new copy of your ID by mail. You can print a temporary ID card from the CHPW Member Center.
What information is on my ID card?
Your ID card contains your member number, name, and information about your primary care provider. Use it as your proof of coverage.
