Programs to help you meet your health goals
Community Health Plan of Washington (CHPW) works with you to meet your individual health goals.
Our Health management programs support communication between you and doctors, as well as coordination between providers, so you get all the care you need. Our approach uses clinical and evidence-based guidelines, which means they have been tested and shown to be helpful.
And, CHPW has regional experts across the state who can guide you to resources near you.
Which health management program is right for me?
We have many different programs to help our members manage their health. If you aren’t sure which one best meets your needs, contact customer service. We’ll be happy to help.
Care Management
Care management addresses your individual health care needs, from food security to helping you get in to see your doctor.
Diabetes Care Program
The Diabetes Care Program is for adults with Prediabetes, Type 1 Diabetes, or Type 2 Diabetes. Case Managers will coordinate your care and help you learn to independently manage your diabetes.

Utilization Management
Utilization Management is a review process that makes sure you are getting appropriate care.
Community Support Services
If you’re having trouble meeting basic needs, our Community Programs team can help. They can connect you with local resources for food, housing, job training, and more.

Health Information Library
CHPW provides an online tool that connects members with important health information. You can browse health topics, watch wellness videos, and much more.
Ensuring Continuity of Care
If you experience times where you are unable to get reliable care or have trouble accessing basic needs, our team can help connect you to the necessary resources.
We make sure there are no interruptions in the care you receive or with the prescription medications you take. We can refer you to additional resources to address your whole health needs. These can include:
- Case Managers
- Transportation and Interpreter Services
- Regional Support Networks for mental health services
- Substance Use Disorder services
- Aging and Disability Services, including home and community-based services
- Skilled nursing facilities and community-based residential programs
- Department of Health and Local Health Jurisdiction services
